Choose your procedure and the state price band that matches where you'd actually have surgery. Be honest about the surgeon tier: "highly sought-after" is the surgeon with a three-month waitlist and a strong revision record, not merely the most expensive. Then compare the low and high estimates against written all-in quotes โ a quote below the low estimate is either a surgeon-fee-only number or a corner being cut.

And a warning worth repeating: the budget tier exists in this calculator because it exists in the market, not because we recommend it. For BBLs especially โ the highest-risk routine cosmetic procedure โ surgeon selection is the one variable that shouldn't be optimized for price.

Three fees hide inside every quote: the surgeon's fee (varies with experience and demand), the facility fee (hospital versus accredited surgery center versus office OR), and anesthesia (typically $600-$1,000 per hour). A budget office-based quote and a hospital quote from a sought-after surgeon can differ by 2x for the same procedure โ and the cheap end of the range is where most of the horror stories come from.
Only when the procedure treats a functional problem: breast reduction for documented back and shoulder pain, eyelid surgery blocking vision, septoplasty inside a rhinoplasty, or reconstruction after trauma or cancer. Purely cosmetic procedures are excluded. Even 'covered' cases often require photos, physical therapy records, and prior authorization.
Choose an accredited surgery center over a hospital operating room (the facility fee drops meaningfully), pick a mid-market metro instead of a coastal one, and use a board-certified surgeon โ never cut that corner. Surgery tourism abroad can cost half as much and carries its own revision and complication economics; if something goes wrong, your local surgeon bills you full price to repair it, and that money was never in the vacation quote.
Pre-operative labs and clearance ($100-$400), post-surgical garments ($50-$150), prescriptions ($50-$200), follow-up visits (usually included for the first year), and any revision surgery. Some quotes bundle anesthesia and facility; others quote only the surgeon's fee, which can be barely half the real total. Always ask for the all-in number in writing.
