Most facility checklists fail the same way: they list everything anyone might ever do, so nothing gets done consistently. The fix is to organize by frequency and let the code-driven dates form the spine โ those are the ones an inspector checks first, and everything else hangs off them. Here's what belongs in each bucket and where the intervals come from.
Because the people doing the work change, and the schedule is what survives. A checklist grouped by system (all HVAC together, all fire together) forces three trips past the same corridor. Grouped by frequency, the weekly round is one ten-minute walk, the monthly round is one longer walk, and the quarterly-plus items become scheduled contractor visits. For a typical office building with all major systems, that works out to roughly 3 weekly tasks, 11 monthly, 8 quarterly, 5 semiannual, and 17 annual โ about 44 recurring items, most of the annual ones owned by outside contractors.
Two things are genuinely weekly in most buildings: activate plumbed eyewash and shower units to flush stagnant water and prove flow (ANSI Z358.1), and verify that unsupervised sprinkler control valves are open, sealed, and accessible (NFPA 25). Buildings with diesel fire pumps add a weekly churn test. That's the whole list โ weekly items need to be few enough that they actually happen every week.
The monthly walk is the workhorse, and it's mostly life-safety:
Quarterly is when a licensed contractor typically re-enters the picture: waterflow alarm tests through the inspector's test connection, supervised valve and tamper switch checks, and dry-system low-air tests under NFPA 25. In-house quarterly work leans mechanical โ AHU belts and bearings, boiler water chemistry, transfer switch exercising โ plus envelope and pavement walks. Semiannual is dominated by kitchen systems: hood suppression service and hood/duct cleaning run every six months under NFPA 96 for most cooking operations, and seasonal HVAC changeover with coil cleaning lands here too.
This is the budget line. A mid-size building's annual stack usually includes the full sprinkler ITM visit with a main drain test (NFPA 25), the fire alarm test (NFPA 72), extinguisher maintenance by a certified technician (NFPA 10), the 90-minute emergency lighting test (NFPA 101), fire door inspections (NFPA 80), backflow preventer tests, the generator load-bank test (NFPA 110), an infrared scan of switchgear (NFPA 70B), jurisdictional boiler inspection, professional roof inspection, and the full ANSI Z358.1 eyewash evaluation. Together these typically run a few thousand dollars a year โ and they're the first thing an insurance carrier or fire marshal asks to see.

These don't need a recurring checklist row so much as a tickler that survives staff turnover โ which is exactly what the checklist is for. Park them at the bottom, date the last completion, and the math is obvious at a glance.
Dates and initials, kept somewhere findable. The NFPA standards are consistent about this: the monthly checks and periodic inspections must be documented, records retained (NFPA 25 asks for a minimum of one year of routine ITM records), and annual certifications kept with the device or in the compliance binder.
